YE²S Student & Parent Portal
The LULAC Youth Educational Enrichment Series program is an online 10-month mentoring and webinar series that serves students between the ages of 13 to 18. The YE²S curriculum features STEAM-focused workshops, one-on-one youth mentorship, academic and career guidance, social innovation, and leadership development.

- Application Packet
- Media Release Packet: All students must complete and submit a signed media release packet with their parent's consent to their program coordinator.
Deadline: November 16, 2020 - Student Entry Survey: All students must complete this form.
Deadline: November 16, 2020 - Mentor Match Up Survey: All students must complete this form. Responses will be used to pair students and mentors for the mentoring component of the program.

- Media Release Packet: All students must complete and submit a signed media release packet with their parent's consent to their program coordinator.
- COVID-19 Waiver: Students must submit a COVID-19 waiver signed by their parent or guardian to participate in any in-person activities hosted by thier designated program site. On-site program coordinator will inform participants if this form is needed and will provide it to students.
- Webinar Feedback Survey: Students are required to complete this form to provide feedback after each webinar. This helps LULAC plan for future webinars.
Deadline: Monthly Deadlines - Student Spotlight: Selected students are invited to complete this form and share their incredible dreams, goals, and achievements.
- Student Exit Survey: All students must complete this form.
